  1. If one had defaulted on some premiums.
  2. If the insured has intentionally instigated or caused the occurrence of the insured risk.
  3. If the loss suffered is not directly related to the insured risk.
  4. If the occurrence of the insured risk does not cause the insured loss.
  5. Violation of an important provision of the insurance contract,such as not acting in utmost good faith or failure to possess insurable interest.
