State the circumstances under which a company would be required to maintain an index of the register of members

Under the provisions of the Companies Act every company with more than fifty members must have an index of the names of the members unless the register is in the form of an index.
It may be in the form of a card.
The index must contain a sufficient indication to facilitate access to a members account.
Changes on the register must be incorporated in the indeed within 14 days.
 The index must be kept at the same place as the register of members.
